Abstract
Abstract Background and objectives: Today, specialists who are responsible for controlling infection in hospital use different disinfectants for instrument used in hospital to prevent from transmitting infection. The spectrum of antimicrobial effect, the condition of exploiting, the time of efficiency, the effect on environment, the destructive effect on metal and plastic materials etc. are different therefore, we did this study to determine the antimicrobial effect of five disinfectants on those special resistant bacterial strains. Material and Methods: In the current study the antimicrobial effects of five disinfectant solutions including Nanocide, Anizosin, Sulfanidis, Micros 10, and Colloid on hospital resistant strains by concentrations of 0.5 and 1 McFarland Were examined and evaluated. The concentration of disinfectants was prepared according to proposed protocol by manufacturing company. The time of contact with bacteria was defined in three times (the proposed time by company, less and more than porposed time by company). Results: Nanocide haven't shown any inhibitory effect on resistant strains in none of the three times but Colloid indicates its effect only in more than proposed time. Anizosin in all three times have inhibitory effect on S.aureus in concentration 0.5 and 1 McFarland. Sulfanidis and Micros to have prevented from the growth of resistant strains in all three times. Conclusion: It has been determined that disinfectant Micros 10 made of ammonium chloride stands as first degree of quality and Sulfanidis made of poly hegza methylen bigouanid hydrochloride stands as second degree of quality with respect to proper antibacterial effect for the purpose of disinfecting. Abstract
Abstract Background and objectives: Blood culture is a critical part of evaluation of Neonate suspected wath Septicaemia. This phenomenon is one of the most important causes of neonates in Neonatal. Material and Methods: This study was carried out on 1470 somples of neonates suspected with bacteraemia , using reutine microbiologicul technique. The samples wene assessed in hazrate Masoomeh hospilal of Kermansha,Iran. Results and Conclusions: of all samples, 112 (7.62%) ane pasitive. Most of the Positive cultures were obtained after 24 hours of incubation in broth mediu. we Confirmed this result by using diseriminafiue culture media the Isolated bacteria are Coagulase-negative Staphylococus(28.6%),Alfahymolylic Streptococus(0.09%), staphylococus aureus(10.7%),Klebseilla(6.2%),pseudomonas(12.5%),moraxella(0.9%),acin eto bacter(13.4%), Alcalingenes(13.4%),protenos(1.8%) and salmonela (0.9%). Conclusion: the frequency of Coagulase-negative Staphylococus Isolated from neonates Blood culture is more than the other micro organisms. Abstract
Abstract Background and objectives: A major problem for labs. esp. medico legal centers is drug test false positive and negative results. Using carbon paper, sodium bicarbonate or oral contraceptive pills (OCP) are common among addict people to make the results negative. Therefore, we decided to evaluate the effect of carbon paper, sodium bicarbonate or OCP on morphine urine test. Material and Methods: We performed this pre-experimental study on the urine samples of all people referred to narcotic drug laboratory of Markazi province during May of 2005. Of 2110 urine samples, the Morphine Rapid Test of 208 samples was positive. Then by means of TLC method, we confirmed the presence of morphine metabolites in 150 urine samples. After that, we divided these samples into three equal groups for adding carbon paper, sodium bicarbonate or OCP. Results: The results show that in carbon paper group, 41 cases are positive and nine cases unclear. In sodium bicarbonate group, 45 samples are positive and 5 cases unclear. In estrogen conjugate group, all 50 samples are positive. Conclusion: According to this study, adding carbon paper, sodium bicarbonate or oral contraceptive pills cannot make negative the Morphine Rapid Test result. Abstract
Abstract Background and objectives: Colonization of nasopharynx by Streptococcus Pneumoniae can lead to pneumococcal disease. This study was performed to determine the carriage rate of nasopharyngeal S.pneumoniae in adolescents and their antibiotic susceptibility and serotype prevalence in Zahedan, Iran Material and Methods: Nasopharyngeal specimens were obtained from 865 adolescents aged 10-19 years old of eight schools in Zahedan and then assessed by standard procedures to isolate S. Pneumoniae. The serotyping was carried out by latex agglutination test, the minimum inhibitory concentration (MIC) of penicillin, as well as other commonly used antibiotics, was determined by a broth-dilution method. Results: Pneumococci were Isolated from 15.7% [136/865, 95% confidence interval (CI) 12.3-18.9] of total samples. Of 136 samples, 119 isolates are typified by the available antisera which the most frequent ones are 1, 19A, 15C, 9V, 11A and 19F. Ninthythree pneumococcal isolates are sensitive to penicillin. The MIC values of antibiotics tested are (μg/ml): penicillin 0.01-4, cefotaxime 0.01-4, ceftriaxone 0.02-128, chloramphenicol 0.08-32, ciprofloxacin 0.06-16, erythromycin 0.01-128, tetracycline 0.08- 128 and vancomycin 0.02-1. Conclusion: A clear diversity is seen in the serotype distribution of the S. Pneumoniae isolates and most of the antibiotic resistant strains belonge to a few serotypes. Abstract
Abstract Bachground and objectives:Streptomyces is the most important genus in Actinomycetes family.The Streptomycetes are widely used in industry producing numerous chemical compounds including antibiotics, enzymes and anti- tumor agents. The aim of this study was to isolate soil-borne Streptomyces producing antimicrobial substances from soil of Golestan province of Iran and to survey anti-fungal metabolities produced by this organism. Material and Methods:In this study various soil samples were collected: ( forest areas of Naharkhoran in Gorgan and Kordkuy’s Derazno, Aghala’s deserts and agriculture lands of Aliabad ) were cultured on Actinomycet isolation agar and Starch casein agar were identified and purified by morphology and biochemistry tests. The activity of isolated Streptomyses against:Aspergillus flavus, Aspergillus, Candida albicans and Malasesia fur fur were studied by Agar Diffusion. Results:Of 120 samples, 24 are Streptomyces(20%) .The frequency of Streptomyces are reported in Aghala (10,41.6%),Derzno (8,33.3%) ,Nahar khoran(4,16.6%) and Aliabad(2,8.3%).Of 24 isolated Sterptomyses,two isolates have strong anti-fungal and six of them have moderate effect.We also see Streptomyses,isolated from desert area, have higher anti-fungal activity. Conclusion:It is recommended two isolated of Streptomyses be identified ana purified. Key words:Streptomyces , antifungal activities, antibiotic

Abstract
Abstract Bachground and objectives: Epidemiological studies have shown that using tobacco products is one of the main factors in forming malignancies in various tissues of the body. There is more than 600 μgr nitric oxide radical (NO°) in gas phase in each cigarette with fresh smoke. Hence, oxidation of nitrogen components in tobacco, more than 100 μgr of atmospheric NO°is produced by smoking, would be transferred to the body without any filtration. We studied nitric oxide levels in the gastric juice of smokers and non-smokers patients with active peptic ulcer. Material and Methods: In this study, 43 smoker patients with active peptic ulcer (14 female & 29 male) referred to gastroenterology clinic with mean age of 45.30±13.16 as case group.Forthy-three non-smokers without peptic ulcer (13 female & 30 male) aged 42.67±16.04, 43 smoker without peptic ulcer (16 female & 27 male) with mean age of 44.58±12.07 and 43 non-smoker with active peptic ulcer (20 female & 23 male) with mean age of 45.37±13.39 were selected as control groups of 1, 2 and 3 ,respectively. The level of Nitric oxide in gastric juice was measured by using Greiss colorimetric method. Results: Comparing with control group one and two, meaningful rise is noticed in mean level of nitric oxide case group (p<0.0001). Mean levels of NO in control group 1, 3 and case group are 4.21±1.13, 5.37±2.26, 7.90±2.12 μmol/L, respectively. Nitric oxide level in case group in comparison with control group 2 dose not show Significant difference (p=0.656). Mean levels of NO in control 2 and case groups are 7.45±1.54 and 7.90±2.12 μmol/L, respectively. Conclusion: It can be concluded that cigarette smoking may be one of the cause of increased level of gastric juice nitric oxide. This increase may be due to component in cigarette smoke and tar. These components can cause DNA damage through oxidation-reduction cycle and consequently increase the risk of malignancies in gastric tissues. Abstract
Abstract Background and objectives: Giardiasis is a parasitic infection of small intestine, with a worldwide distribution and the prevalence of Giardia in different parts of the world varies between 1 to 25%. Plants have the vast range of antimicrobial and antifungal activity that can be identified as alternative treatments for bacterial and parasitic pathogens, the same as Giardia. In this study, the methanol extracts of eucalyptus plants, Satureia hortensis and Heracleum glabrescens, on Giardia cysts were studied in vitro. Material and Methods: The cysts were isolated from the feces using a modified Bingham. After counting by Hemusytumetr, they were placed near by 200 mg / ml, 100 mg / ml and 10 mg / ml of the extracts prepared by DMSO for 30 and 60 minutes. Then, the number of dead and live cysts was counted under a microscope. Results: the fatality effect of the extracts in 60 minutes is higher than those of 30 minutes. The methanol extracts of Satureia hortensis, Eucalyptus and Heracleum glabrescens with the dilution of 200 mg/ml in 60 mins have the fatality effect of 84/3%, 63/3% and 44%, respectively. The highest fatality(84.3%) on Giardia cysts is related to Satureia hortensis with the dilution of 200 mg/ml in 60 mins and the Lowest(27%) is related to Heracleum glabrescens with the dilution of 10 mg/ml in 30-minute period. The significant relationship between the plant type and the fatality of methanol extracts is observed. Conclusion: the methanol extracts of Eucalyptus, Heracleum glabrescens and especially Satureia hortensis have anti-parasitic effects in the laboratory conditions. Abstract
Abstract
Background and Objective: The high occurrence of early childhood caries (ECC) is one of the most common problems in children dentistry. Streptococcus mutans and lactobacilli.spp are closely associated with the development of early childhood caries (ECC). Therefore, the aim of this study was to investigate the frequency of streptococcus mutans and lactobacilli.spp in 3-5 –year- old children with and without dental caries.
Material and Methods: This case-control study was conducted on 60 children aged 3 -5-years, without any history of systemic disease, who had not received any antibiotic therapy and fluoride usage during the last month. The cases were divided into three groups of early childhood caries, usual caries and caries-free. The infected dentin samples were collected from cervical and proximal in ECC and usual caries group, respectively. Also in all the three groups, the samples of dental plaque from buccal surfaces were collected and immediately immersed into Brain Heart Infusion (BHI) broth medium. After that, the diluted sample was plated onto MitisSalivarius agar (Difco) for detecting streptococcus mutans and Rogosa agar (Difco) for detecting lactobacilli.spp. Data were analyzed by Chi- Square and ANOVA.
Results: of the samples taken from dental plaque, S. mutans is observed in 90% of ECC, 80% of proxymolcarries and in 25% of caries-free individuals. Based on the results, the presence of S.mutans in the group of caries is significantly higher than that of without caries, but there is no significant difference between the two groups of caries. In addition, lactobacill.spp is isolated from 60% ofthe samples, but there is no significant difference between two groups of caries and caries-free. The average of decayed, missing and filled teeth (DMFT) index in the group with early childhood caries (6.95±1.572) and usual caries (5.80±1.105) is different significantly. Pearson correlation test shows that there is a significant relationship between caries experience and the amount of S.mutans, but there isn’t this relationship withlactobacill.spp. Conclusion: The findings show that the children with early decay have the higher level of S.mutans in their dental plaque. The most common strain isolated from dentin, without consideration of their location, is S.mutans.

Abstract
Abstract
Background and Objective: One of the diabetes complications is the tissue damage caused by the imbalance of oxidants and antioxidants (oxidative stress). The aim of the present study was to evaluate the activity of two antioxidant enzymes -superoxide dismutase and catalase- in the serum of streptozotocin-induced diabetic rats.
Material and Methods: This investigation was conducted on adult male rats assigned to diabetic and control groups. Diabetes was induced by a single intraperitoneal injection of streptozotocin. Seven weeks after diabetes induction, glucose concentration, superoxide dismutase and catalase activities of the serum were assessed.
Results: Glucose concentration of streptozotocin-injected animals was significantly higher than that of control group (P<0.001). The level of Serum superoxide dismutase and catalase activities in diabetes group were significantly higher than those in control group (P<0.01). There was a positive significant correlation between glucose concentration and superoxide dismutase and catalase activities (P<0.001).
Conclusion: The high activity of antioxidant enzymes in diabetic rats is probably due to compensation responses to oxidative stress produced by high concentration of free radicals. It seems that the higher glucose concentration, the greater compensatory responses.

Abstract
Abstract
Background and Objective: Metabolic syndrome called a cluster of several metabolic disorders is associated with increased risk of cardiovascular diseases. Genetic differences in leptin receptor gene are related with the concentration and activity of leptin in that these discrepancies can influence lipid levels. We aimed to determine the association between the leptin receptor gene polymorphism on serum lipid profile and leptin activity in metabolic syndrome patients.
Material and Methods: This case-control study was conducted on 200 patients with metabolic syndrome and 200 healthy individuals. Polymerase Chain Reaction (PCR) and Restriction Fragment Length Polymorphisms (RFLP) were used to determine genotypic distribution and allelic frequencies of polymorphisms, respectively. The plasma leptin activity was measured by a kit in a fluorescence spectrometer, and Lipid concentration by routine biochemical and enzymatic assays.
Results: Two groups had significant differences in all measured factors such as lipid profiles, fast blood sugar, waist circumference, blood pressure and leptin concentration (P< 0.05).
Conclusion: Given that the two groups had significant differences in blood and body measurements, no role of K656N polymorphism was observed. Overall, Lys656Asn (K656N) polymorphism of leptin receptor gene is not associated with serum lipid profile and leptin activity with metabolic syndrome.

Abstract
Abstract Background and Objectives: Contamination of environment to lead and copper is rising due to human activities. One of the best methods to remove heavy metals from the environment is bacterial remediation. This study aimed to isolate bacteria and investigate the mechanism of lead and copper bioremediation. Material and Methods: Heavy metal resistant bacteria were isolated from contaminated wastewater samples. The isolates with high resistance to lead and copper were selected for further studies and bioremediation was assessed by atomic absorption spectrophotometer. To determine the functional groups to remove metals, FT-IR was employed. In addition, plasmid curing was studied to determine the location of the genes that are resistance to heavy metals. Results: Ten bacterial isolates that are resistance to heavy metals were isolated. Among these, MKH3 with the highest remediation activity removed %90 lead and %92 copper from the growth medium. The absorption mechanism of MKH3 indicated that the functional groups such as carboxyl, amide, carbonyl and hydroxyl were most effective for removal of heavy metals from the growth medium. The results revealed that heavy metal resistant genes may be located on plasmid DNA. Furthermore, molecular identification demonstrated that MKH3 was similar to Enterobacterhormaechei with 98% homology. Conclusion: Bacterium isolated from a contaminated site showed the ability to remove a high amount of lead and copper. Thus, MKH3 could be useful for the bioremediation of heavy metals, particularly lead and copper, from industrial wastewater and contaminated sites. Abstract
Abstract Background and Objective: Streptococcus pneumoniae is the most common cause of acquired bacterial infections in the respiratory system. In recent years, a high incidence of pneumococcal resistance to different antibiotics has also been appeared. This study was conducted to evaluate the in vivo and in vitro resistance of pneumococcal pneumonia to ceftriaxone, azithromycin and co-amoxiclave in clinical setting and laboratory. Material and Methods: In this single-blind clinical trial study, the participants were the patients with the diagnosis of pneumonia referred to infectious diseases clinic in Vali-e-Asr hospital of Birjand university of Medical Sciences, October 2012 - April 2014. The patients were randomly allocated to one of the three therapeutic regimes including azithromycin, ceftriaxone, and co-amoxiclave. After 48-72 hours that the infection was confirmed by paraclinical findings, the patients with pneumococcal pneumonia remained in the study and their in vivo and in vitro resistance to the above mentioned antibiotics were compared. Results: The most in vitro drug resistance was to co-amoxiclave (41.5%) and the least to ceftriaxone (20.8%) (P>0.05). For In vivo, the most resistance was to azithromycin (47.4%) and the least one to ceftriaxone (6.7%) (p<0.05). The agreement coefficient between the laboratory antibiogram test and the clinical responses to therapeutic regimes of azithromycin, co-amoxiclave and ceftriaxone was 0.25 (p=0.26), 0.46 (p=0.02) and 0.44 (p=0.04), respectively. Conclusion: With regard to the demographic characteristics of the patients in this study, the resistance of Streptococcus pneumoniae to ceftriaxone is less than that of co-amoxiclave and azithromycin in both clinical setting and laboratory. Abstract
Abstract Background and Objective: Group B streptococcus is one of the agents leading to infantile blood infection and meningitis as well as aggressive infections in pregnant women. The vast use of antibiotics in order to prevent from the diseases caused by this bacterium has led to some concerns about the emergence of drug resistance in Group B streptococcus. Thus, we aimed to investigate Group B streptococcus drug resistance patterns to aid appropriate drug prescriptions. Material and Methods: The study was conducted on 268 pregnant women. Having been transferred to Todd Hewitt broth, the samples were cultured on blood agar and the identity of Group B streptococcus was confirmed through biochemical tests (gram stain tests, the catalase test, CAMP, and sodium hippurate hydrolysis). Results: Of 268 samples, 14 (5.2 percent) showed positive Group B streptococcus culture. All cases (100 percent) were resistant to penicillin, eight (57.15 percent) to erythromycin, 13 (92.2 percent) to clindamycin, three (7.1 percent) to Cefazolin, seven (50 percent) Ceftizoxime and four (28.57 percent) were resistant to Cefotaxime. Three (21.4 percent) were semi-sensitive toward Cefazolin. No resistance was observed toward ampicillin or Vancomycin. Abstract
Abstract Background and Objective: Increased antibiotic resistant strains and inadequacy of current vaccines against pneumococcal infections necessitate the study of novel protein antigens. It seems that minor autolysin of Streptococcus pneumoniae may have antigenicity. Thus, we aimed at cloning its gene for the first time. Material and Methods: After DNA extraction of Streptococcus pneumoniae (ATCC 49619), Specific primers were designed for amplifying minor autolysin gene fragment, using PCR. The purified gene fragment was inserted into pET21a vector and was transformed into bacterial competent cells by heat shock technique. The presence of gene and absence of mutation in the recombinant vector were checked out with sequencing and enzymatic digestion methods. The gene sequence was finally analyzed by bioinformatic tools. Results: The gene of minor autolysin was cloned successfully and the result of enzymatic digestion was the indication of complete isolation of this gen from plasmid. . Bioinformatics studies revealed that the mature protein was lacking signal peptide and the gene encoded 318 amino acids with a molecular weight of 36.4 kDa. Conclusion: The presentation and characterization of novel antigens such as minor autolysin could help us with finding new approaches for preventing and controlling pneumococcal infection. Abstract
Abstract Background and Objective: Cryptosporidium is a common protozoan causing diarrhea in human, specifically in children. Hence, we aimed to investigate the prevalence of this protozoan among diarrheic children hospitalized in Gonbad Kavus in 2011. Material and Methods: Three stool samples were collected from diarrheic children in two hospitals of Gonbad city and a relevant questionnaire was filled out for each child. The stool samples were concentrated by formalin ether method, and the infection was assessed by modified acid-fast staining method. Results: Of 547 children, 27 (4.9%) were infected with cryptosporidiosis. There was no significant relationship between the amount of infection and gender and habitation area (urban/ rural). The infection rate was significantly prevalent in 2-4-year-old children (P=0.013). The most and the least infection rate were observed in spring and winter, respectively (P< 0.0001). There was a significant association between the disease and keeping animal (P= 0.041) Conclusion: The prevalence of cryptosporidiosis in diarrheic children in Gonbad is almost equal to other regions of the country and keeping animal and spring season may be considered as the risk factors for the disease. Abstract
Abstract
Background and Objective: Leptospirosis is a zoonosis infectious disease that is prevalent in tropical and subtropical regions and is caused by the pathogenic serovars of leptospires. Hence, we aimed at investigating the prevalence of antibodies against these bacteria in the blood samples of suspected leptospirosis.
Material and Methods: the human serum samples (N = 130) were obtained from patients clinically suspected leptospirosis. The Serum level of IgM antibodies were studied by ELISA kit (PrioCHECK) in Razi Vaccine and Serum Research Institute (Karaj), 2011-2012.
Results: Anti-leptospira IgM class was observed in 21(16%) samples. The relative distribution of the disease was reported in men (80.95%), women (19.04%), and farmers (30.95%) and in 20-40-year group (57.14%). Contact with contaminated water was the most common cause of infection (52.38%) and fever was the most common sign of Leptospirosis (72.2%).
Conclusion: Due to the occurrence of anti-leptospira antibodies in 16% of suspected cases, it is recommended that routine ELISA be done at least in major diagnostic centers.

Abstract
Background and Objective: T3 resin uptake test indicates the availability of thyroxin binding globulin (TBG), which is the carrier of a large amount of T3 and T4 in blood. We aimed to study the relationship between serum T3 Resin Uptake and serum lipids profile.
Material and Methods: The participants aged 14.80 ± 38.29 were both males (59 29.5%) and females (141 70.5 %). T3 Resin Uptake was measured by Immunoassay method and serum lipid profile components (cholesterol, triglycerides, HDL-C, LDL-C, and VLDL) were measured by the kit.
Results: Significant negative correlation was observed between serum T3 Resin Uptake and cholesterol, triglycerides and LDL-C (P≤ 0.05), but that was not the case for HDL-C and VLDL.
Conclusion: Serum T3 Resin Uptake can be evaluated to assess the risk factors related to cardiovascular disorders.

Abstract
Background and Objective: Propionibacterium acne is one of the main causes of acne. Due to the spread of drug resistance, it is not responsive to treatment. This study aimed to determine antibiotic sensitivity of strains of the Propionibacterium acne.
Material and Methods: seventy samples of acne lesions were collected to study the presence of Propionibacterium acne. Microbial Culture technique was used to detect and identify Propionibacterium acne. Antibiotic resistance of the isolates to the antibiotics of Doxycyclin, Azithromycin, Erythromycin, Tetracycline, Clindamycin and Trimethoprim sulfamethoxazole was studied by Antibiogram method.
Results: Of 70 samples, 14 (20%) were positive for Propionibacterium acne. The results of phenotypic test were confirmed using molecular method. Rate of resistance to Azithromycin and Erythromycin (50%), Clindamycin (35.71%), Trimethoprim sulfamethoxazole (28.57%), Doxycycline and Tetracycline (14.29%) was determined.
Conclusion: Outbreak of antibiotic resistance to Azithromycin, Erythromycin, and Clindamycin is high. Since the Propionibacterium acne is sensitive to Doxycycline and Trimethoprim Sulfamethoxazole, it is recommended using them to treat acne.

Abstract
Abstract
Background and Objective: This study aimed to investigate the antimicrobial susceptibility of the most common pathogens in hospitalized neonates in Intensive Care Unit.
Material and Methods: In this one-year descriptive study, 150 blood samples of neonates in Intensive Care Unit of Bahrami hospital of Tehran were divided into two groups of early onset sepsis (the first 72 hours of life) and late onset sepsis (after the 72 hours of life). After isolating and identifying of bacteria, their antibiotics susceptibility was studied by Kirby- bauer method in accordance with CLSI guidelines.
Result: The most isolated organisms were Klebsiella pneumoniae (41.3%). Coagulase negative staphylococcus was the cause of early onset septicemia and Klebsiella pneumoniae of both early and late onset septicemia. The highest susceptibility in gram-negative microorganisms was shown to Ciprofloxacin (89.6%) and in gram positive to Vancomycin (82.8%).
Conclusion: Gram-negative bacteria are the main cause of contamination in NICU and ciprofloxacin is the most effective antibiotic. Thus, it is imperative that NICU should be extremely controlled.

Abstract
Background and Objective: Cryptosporidium parvum is a zoonotic protozoan parasite causing diarrheal cryptosporidiosis. Numerous outbreaks of cryptosporidiosis have been reported worldwide. The transmission via milk, water and raw animal products is one of the important ways. The aim of this study was the identification of hsp70 gene in Cryptosporidium parvum in raw cow’s milk samples.
Material and Methods: In this cross sectional study, 38 raw cow’s milk samples of bulk tank were randomly collected from traditional and semi industrial cattle farms in Isfahan. To identify the protozoa in milk samples, the extracted DNA was evaluated by Nested polymerase chain reaction (PCR).
Results: Based on Nested polymerase chain reaction, 2 samples (5.26%) were infected to Cryptosporidium parvum.
Conclusions: The contamination of milk with Cryptosporidium Parvum is less than that of the other foodstuffs. Thus, it is necessary to reduce food contamination and to have appropriate health education programs.
